Crisis Debrief Application 

To apply for a Crisis Debrief, fill out a Personal Debriefing Retreat application form, stating in the "Reasons for Requesting Debriefing" that you are applying for a crisis debrief. Then send us a separate email that explains the following:

  1. What happened
  2. When it happened (If it was more than 3 months ago, please apply for a Personal Debriefing Retreat in the normal manner.)
  3. Where it happened
  4. Who was involved
  5. Your role in it

Note: If you don't have your computer set up to automatically connect to email from a link, then right-click on "separate email" and copy and paste the email addresses into your email program.
